Payment for Vision Care Services
Full payment is due at the time of service and before any product is ordered unless specified otherwise. We accept payment through cash, personal check, credit card (Visa, Mastercard, Discover Network, American Express, Diners Club), and mobile systems (Apple Pay, Android Pay, and Google Pay). If you have a flexible spending account (FSA) or health savings account (HSA), you may use these for any professional services, co-payments, prescriptive glasses and sunglasses, and contact lenses.

Medical Documentation and Accommodation Requests
Our practice is committed to providing appropriate medical care to our patients based on their individual medical needs and the scope of services provided by our practice.
Disability, Accommodation, and Functional-Capacity Documentation
Please be advised that our practice does not complete, prepare, or sign letters, forms, certifications, or other documentation intended to:
- Support a request for a workplace, educational, housing, or other reasonable accommodation;
- Establish or certify disability status for purposes unrelated to the medical care provided by our practice;
- Support disability, leave, benefits, insurance, legal, or similar claims; or
- Provide opinions regarding an individual's eligibility for accommodations, disability benefits, or other third-party determinations.
These types of evaluations and opinions are outside the scope of the services offered by our practice. Patients seeking an independent assessment for disability, functional capacity, reasonable accommodations, or related claims should consult a physician or other qualified healthcare professional who specifically provides these services, including providers who perform independent medical examinations (IMEs), when appropriate.
Legal or Court-Ordered Documentation
The foregoing policy does not apply when our practice is legally required to provide information, records, testimony, or other documentation pursuant to applicable law, regulation, subpoena, court order, or other legally enforceable process.
When legally required to do so, our practice will comply with the applicable legal obligation and provide only the information or documentation required by law or the applicable order or process.
Medical Records
Although we do not provide the documentation or opinions described above, patients may request a copy of their medical records for use by another healthcare provider or for other lawful purposes.
Medical records will be released in accordance with applicable law and our office procedures upon receipt of an appropriately completed and signed authorization or medical release, when required.
Our records reflect the care and services provided by our practice and may be made available to another qualified healthcare professional for purposes of an independent evaluation or other assessment.
